For my sims was during the woohoo challenge in February. I absolutely did not want any poof babies - so it sort of peeved me it happened, as I normally am not allowing any woohooing period or romancing - but was foolish enough to try the challenge to try and help everyone get some free stuff.
I have not had any woohooing of any kind in my game since. As it is I had to turn off aging so that poof baby does not poof into a 10 year old. i am hardly playing Sims 4 at all now. But my daughter - in -law also claims she has had at least 2 accidental pregnancies in her game just since the last game update. She is a very cautious player so there is no way she hit the wrong button as she does not like children in her game at all in Sims 4. She doesn't want families in Sims 4 without toddlers. So it is still happening.
JUst so you know - neither of us use any mods or CC, so it is happening due to game design and not because of mods.
