For those having this issue, try to have a Flirty conversation go long enough that the conversation type (listed in the top center of the screen) get to a high level of Flirty, like "Steamy conversation" or better and make sure no other sims are participating in the conversation except for the proposer and proposee.
I have this issue as well, but literally always can get the proposal accepted if I run the flirty conversation up to a high flirty level. The reason for making sure other sims aren't injecting themselves into the conversation is because that tends to lead to a flirty conversation turning into an 'awkward conversation', which pretty much is a guarantee for rejected proposal, at least for me.
