I was getting this issue a lot as well - my Sims were originally showering in CC clothing so I used S4S to do a batch fix. Then they started showering in EA clothing and I was stumped - I had no idea what could be causing that to happen but it kept on occurring, but only on one or two Sims, not all of them.
I ended up downloading this mod that allows you to copy outfits from other Sims - so my Sim went to the gym and copied one of the other Sim's nude outfits, and the issue went away.
