For one thing, this is definitely more of a autonomy issue than a jab at the LGBTQ+ community. Once you've played a sim, the game shouldn't be destroying what you've created, even if you stop touching the sim. More so - the sim shouldn't be getting into romantic relationships, making enemies, ending relationships - without the player doing so.
Transgender sims are an entirely different issue, as the only real way to incorporate that into the game is a way that's kind-of offensive to trans people. Someone who is trans has *always* been the gender they identify as - regardless of their body and what they were living as - and if you incorporate a way for sims to "change gender" it's a little unsettling and could offend people pretty easily. A better concern would be more open-ended and neutral options in CAS - the body types should be editable in a non-gendered way - female sims should be offered options of less curves, flat chests (not even just a trans issue - plenty of cisgender women don't have "feminine" breasts!) and a more boxy, straight figure, and vice-versa - men should be offered options of more curves, and a chest without pecs. It's more of an issue of sims subconsciously being pushed towards traditional options because the non-traditional options are poorly implemented (aka, no torso curves on a female sim and torso curves on a male sim all make their stomachs look weird and make the clothing fit illy).
