Haha, the game shows no mercy! My late Duchess Claudine de Cavalier was unlucky, too, but then it was situational attire and not something I had forgotten to check. I hate it when the game dresses my duchesses up like this! She also got numerous piercings. I'd prefer a game with a better fashion sense, or a game that allowed me to play nobility that are dressed according to their high standards ;)
I hear you. Sometimes the game shows no mercy. Just recently, and for the very first time, my adult male Sim suddenly had back hair I didn't give him. I have been amused many a time when NPCs walk in front of my Sims' house dressed just as strangely as your Sim in the picture you shared. Oi!
We must see it as entertainment. It makes it easier to deal with. Not sure my duchess would agree, but she died a while ago anyway, hopefully not "killed by random situation costume" ;)
