I am guessing you haven't seen any npcs with ailments? My thought would be to try to find them and expose yourself but, if everyone had high motives they're probably not getting them. I read somewhere that if you let your sims motives get low, they will be more susceptible and can even get an ailment without being exposed. Not sure if you read that and if not maybe it will help your quest :)

No, I've not seen any NPCs with ailments. 🙁I'll send my sim out with low motives and see what happens.

Wow, honestly, even if I don't leave the home lot I get quite a few. I don't see how you haven't gotten any. Unless you play a 1 Sim household, never see other Sims and never leave XD

Have not gotten any ailments either, besides from ones I've given to other sims as a fairy 😅
then again yeah, 1 sim household and I'm keeping those needs full cause of my perfectionism 😆

I got aliments when I made new games with my single sim in the werewolf world on the 30X30 lot. Every new game I started there my sim got an ailment after the welcome wagon came. I can't remember the werewolf's name but it was the father? werewolf, not the daughter? werewolf that came over for the welcome wagon. I think he infected my sim.
ETA. Other than the 2 times that my sim got an ailment in the werewolf world she hasn't got any and I've started new games in Newcrest and several in Innisgreen.
