Lucky. My sims NEVER gain weight. Not even my pregnant mommies! I'm trying to create stories here. :(
Let them eat a lot of cakes, there is a calories system, they gain weight depending on what they eat and the average muscles they have (a muscular sim gains less weight than a non muscular one, men sims gain less weight than women, it's a complex system)
