My sim Sat there for three Sim hours, making a Weird Moaning noise, while Scrubbing the toilet, it still had the Green smoky stuff coming out? could someone of taken such a nasty dump its permanent...

Solution - sell those toilet, buy new ones but don't upgrade till the child is born.
That's how it happened with me. She puked in a toilet and it became impossible to clean after that.
Wasn't a big deal though, I just replaced the toilet and upgraded it again.
