Friendly introduction with the doctor didn't work?
The doctor leaves early on some days. When he does, I always just did everything except for transfer the patient case. Eventually, I did end up getting fed up with him leaving early and leaving me with all of those patients, so I just promoted myself to Assistant Nurse. The higher careers DEFINITELY help. You get more time, and eventually can diagnose patients yourself.
To promote yourself, use ctrl + shift + c to bring up the cheat window.
Then, type careers.promote Doctor
Do this until you reach Assistant Nurse or whichever level you want.
To demote yourself, use careers.demote Doctor. This will not demote after you branch off into different careers.
