You don't, but it will still show up because like in real life there is some sim who is a litterbug and really doesn't care. I finally got the island cleaned up 100% as well, but now I don't go an search for it as I did before. The island seems to be ok even though my sim's have slowed down in their efforts. But I do still see other sims (I'm not playing) contributing towards the overall health of the island.
