Trouble Reading Disc?

Does anyone else have the problem with their discs being read on PS4? I place the disc in the drive and it has trouble reading it. I usually get the "Unrecognised Disc" message yet slip any other game disc in the drive and it runs perfectly.

It typically takes several tries to get the disc to be read. I once cleaned the disc, slipped it in and it ran first go. Since then, slip the disc in and it takes several goes to get it to be read. Eventually it loads.

I tried it in my sister's console today and it worked perfectly, first go.
