I wonder if there will be a way to keep people in the waiting room until it is their turn or if they will be wondering in and out of exam/surgery rooms? Perhaps a new way to lock doors? You can't lock...

You can't technically have an "at home" business. You have to go to the menu and buy an empty lot or one of the designated vet clinics and run the business on that lot. Granted, you CAN build a place for your sim to basically live on that lot (say, an apartment above the clinic) but their "home" will always be their original lot.
