I consider sulani to be too far away from the other worlds. I have however played a sim living in sulani and working as a doctor. For the most part when I'm playing on the active career lot I stay busy and don't focus much on the world outside. I think it could be easy enough to imagine your sim is working in a city like Honolulu which could be located close to Sulani.
