For story telling you could build a new hospital located anywhere and go there on those days you choose to work from home. By placing the hospital objects there your sim will be able to perform some tasks and build skill/performance. There won't be random patients and other employees, but at least the environment would be right, and then your sim could work in a hospital "elsewhere" to benefit from more career features.
