I have had two sims die from cooking scrambled eggs. And one didn't die, but lost her kitchen. YMMV
I went straight from reading this thread to reading about a Sims 4 kitchen fire on Twitter. I had a kitchen fire in the game in … March? Early April? in a fresh save. I suspect it's a lot like twins etc., that there's a degree of chance and we don't all draw the winning random number with the same frequency as each other.
The fireplace thing was a definite change that we were told about. But I'm still seeing people post about other fires. They don't seem to be connected to stopping cooking but to be random low-skill incidents.
