Do you have bad lot traits or unskilled sims doing the cooking? ... and yes I think there may have been a little tweak at some point in the last few patches. I've seen some more than usual but really not a lot unless you use certain things to cook on or they don't have skills.. or you give them miserable traits and have them cook while in a bad mood.
