Mostly it only happens when I have my sim visit other households.. or I use the always welcome trait a lot and see fires because of the things they do. It's usually that little cheap grill, but can be other stoves/grills too. Then again I have my sim cook often to head off trouble (large plates) then store them in the fridge. Usually they eat what's there, sometimes they want to cook anyway because they are stubborn or stupid or just want that hotdog really bad.
To me though it's easy to head off the problems. No cheap stoves or fridges, especially at low skill levels, skill up to cooking 2-3, get inspired before cooking if possible or at least don't send them to cook tense, angry or uncomfortable. Don't have bad lot traits. And like I said before they live off salads and BLT's for a little while (group meals).
Otherwise though it makes it easier to start a fire or at least have some chaos now and then.. so I wouldn't want it tweaked back down personally. Still haven't had a sim die in this situation.... without set up.
