Have you heard of Remus Sirion? He has a lot of natural looking facial features. I'll link you straight to his navigation. Might take a minute to load, as his stuff is high quality. His skins aren't too bad. I only use his male fantasy skin for male vampires though.

Most skins I find online don't look well in-game (even with HD installed). There's a good male and female skin I found by Ms Blue on TheSimsResource. They are called Markus for the male and Mirabelle for the Female. You can check out her work here:

I don't have any clothing recommendations. Like you, they've all ended up looking wonky in-game as well. I use default/game clothing.
