It seems to happen with Pre-Seasons CC and the way I found it was creating a new sim and putting that same CC on the sim and saving him to my library. Once I did that I used Sims 4 Tray Importer to capture the Sim from my library and view the CC. Just start at the top and click each piece of CC going down list. I found several offending CC hidden in 'merged' package files I had picked up along the way.
