What expansions, game packs, and stuff packs do you have installed? All expansion packs, all game packs except Star Wars. Some stuff packs: Nifty Knitting, Tint Living, Laundry, Toddler, Vintage Glam, Perfect Patio, Cool Kitchen, Spooky, Romantic Garden, Kids Room
Steps: How can we find the bug ourselves? Would be tough. Go to San Mishuno, pre-made sim (Salim Benali). Play awhile and save. Exit to Manage World. Try to choose this sim to play.
What happens when the bug occurs? I saved and went to Manage Worlds. When I wanted to come back, there was a name (Benali) but instead of his picture, there was blue square blinking. When I tried to approach form Manage Households, there was his tile and a name, but instead of picture red X. So I tried to move somebody to this household, but when I did she was the only sim there, and the original "disappeared" sim (and his cat) were gone. In the Benali household should be only Salim Benali. His name still shows up but the picture is now "borrowed" from neighbours next door.
What do you expect to see? I expect to see my sim picture when I am loading a lot, and most of all that sim to be there when I do.

Did this issue appear after a specific patch or change you made to your system? No
Sim has disappeared. His name is showing as parcel owner but his picture disappeared and I can't enter the lot. He also disappeared from contact list of everybody who knew him.
