NennaRC In Neighborhood Stories (NS), ticking the heart option only allows relationships to change in the background for the category of sims you have the heart ticked for. Example: If you have the heart ticked for just the Other tab sims, then background romantic relationships will be limited to sims in that tab. Same goes for sims in the My Sims tab if NS is on and the heart is ticked for that category. If it is ticked in both tabs, then background romances can occur and change between played and unplayed sims. Now, this doesn't include what happens when the NPCs and active sims are in the same zone and the NPCs autonomously do romance socials with your sims. That's a completely different thing unaffected by NS.
The Game Options setting you mention is for the phone calls. All of those require the player to answer by clicking on the popup. There is one that will get your sim married so you have to pay attention to what the popups say to make sure you actually want whatever the phone call is about to happen. If you turn both of the Game Options off (friendly and romantic) off, you just don't get the phone calls anymore.

Ok, so it actually can happen in the background without any mod causing it. I will need to keep these unticked on the townie/other sims side.
So, this thread belongs under game issues, not under mods and custom content.
Now when that is sorted, my second question remains also. Is it by game design that it's obviously possible for my already married sims to get married to yet one or several other sims/townies while still remaining married to the spouse they already have? I don't like this and I don't like that relationship status can change through a phone call. Who gets engaged or married over the phone?

I accidentally came across your thread and questions. I play a completely vanilla game and do not use mods or cc. I can tell you that the game absolutely won't have sims get married on its own with NS. That is actually a feature I would have liked to have but the devs were careful not to allow it for players like yourself who want more control over the marriage process. The phone call you mention is for players like myself, who did want marriage, to have an option for it to happen. It was a way to try to allow all players to have options for what they want to accomplish and I applaud their efforts to try and balance the game for everyone.
If you are seeing sims marry autonomously, some mod you are using is allowing this to happen, whether or not you know that is a feature. I would suggest that you give your mod list to luthienrising so she can try to help you figure out which mod is causing the undesired behavior.

NennaRC And is the marriage happening ONLY when you click on a pop-up? Have you tested without mods, in case a mod is broken in a way that it's making this happen even if it's not a mod feature?
