If you play a modded game and happen to use a zone limit mod to allow more sims on community lots, this "glitch" can occur on a penthouse/apartment shell lots once your building reaches its zone limit. Basically what happens is the elevator queue becomes backed up, so you'll see sims literally standing in line waiting to use the elevator. If you wait long enough you'll even start to see the sims eventually clear themselves one by one.
But unfortunately as soon as one clears, another sim is spawned.
The resulting traffic jam causes routing issues for active sims to use the elevator. The only work around is to use the teleport sim cheat.

I was also able to by-pass the routing issue altogether by installing weerbesu's "Visitors Not Spawning in Front of Venues" and "No Empty Venues When Arriving" mods. With these two installed, my elevators are now back fully functional.
