If you play a modded game and happen to use a zone limit mod to allow more sims on community lots, this "glitch" can occur on a penthouse/apartment shell lots once your building reaches its zone limi...

I was also able to by-pass the routing issue altogether by installing weerbesu's "Visitors Not Spawning in Front of Venues" and "No Empty Venues When Arriving" mods. With these two installed, my elevators are now back fully functional.
