    @Nugett2024 What packs do you have? Has she been abducted (UFO/spaceship)? When this happens a Sim can return pregnant sometimes?

    I don’t have the ufo. Have vampire, eco lifestyle, cottage living and tiny living

    @Nugett2024 Did you ever have Mods/CC in the past?

    Is your game fully updated:

    How to update your game:
    • open Origin
    • click ‘Game Library’
    • right-click Sims 4 > Update Game

    You could also try running a game repair. It may not help, but it couldn't hurt:

    How to run a game repair:
    • Open Origin, click ‘Game Library’ then right-click Sims 4 > Repair Game
    • Let it run until it has completed all repairs. It will do the base game first, then run through each of your DLCs (Expansion Packs, Game Packs, Stuff Packs and Kits).

    I was chatting with other helpers who wondered if it could have any connection with the new 'neighborhood stories'.
    Did your sim get any phone call from any neighbor or NPC sim about pregnancy, expanding family, or is there any romance/flirting going on with others that you know of with this sim?

    Edit: I saw this, but it's marked fixed and it seems unrelated as it seems to be about asking advice then the other sim becoming pregnant:
