Is this happening for all of your Sims or just this couple? What are the traits of the Sims? (Some traits make romance more challenging &/or different, but I've never dealt with any that gave no romantic options, unless the relationship and romance are too low.) What is their friendship & romance (pink) bar level? Is either of them not interested in the opposite sex, in a different relationship, etc.? Do you have Mods/CC? If so, did you test if the issue is gone without them? (Directions for this in spoiler tag , if you need them.)
SpoilerExit the game. Move the Mods folder from your "The Sims 4" folder [Documents > Electronic Arts > The Sims 4] to your desktop (save it for reference) and delete the localthumbcache.package file in your “The Sims 4” folder. Then check in a new game save to see if this issue is still occurring. You can then test without saving – or in a new vanilla game save – in order to identify which CC/Mods are causing the issue. Then remove or add updated version(s) of the Mod/CC causing the issue.This is happening to both of the sims in the relationship in that world however if I go to another saved game it’s different. If I try using romance interactions in another saved game it gives me a few more romance options (as seen in picture) but it’s still not the usual options I would normally get. And yes, I do have mods and CC. I’ve tried taking out any mods and CC that may be causing this but it’s still not working.
@janelleyay11 Mods/CC that are causing a conflict/glitch/breaking functions in your game don't necessarily need to have anything to do with the type of interaction/function you're having an issue with. Also, CC &/or Mods that are in the game, and sometimes that have been removed but have left cache in the game, can affect your game in ways that may or may not make sense. Even installing the latest version of a Mod/CC does not guarantee that there is not a conflict that is affecting the game due to a more recent game update or due to a Mod/CC update that did not fully address issues.
Please try the following as stated. If you did any of it before, please do it again with all of the other directions, and see if the issue's gone. You don't need to save without the Mods/CC in the game you test in, just confirm if it's gone. If it's gone, you can troubleshoot finding the Mod/CC causing the issue in a new, vanilla game save:

I am also having the same issue! I have followed all the guidance and still can not access romantic option even when all my mods have been removed and the cache has been deleted and have started a new game?
- @ktsalsa Is one of the Sims a Teen and the other older? Have you checked that they aren't related even if in different households? If none of these are issues, try a Game Repair. When removing cache, make sure you get the file called localthumbcache. And remove *all* the content of the mods folder, including custom content.
