Let's give the following steps a try:
Run a full power cycle on your console by following these steps:
- Turn off the Xbox console by pressing the Xbox button on the front of the console for about 10 seconds. This will shut down your console completely.
- Unplug the console's power cable. Wait 30 seconds so the power supply is reset.
- Plug the console power cable back in.
- Press the Xbox button on your console to turn on the Xbox.
If this doesn't help, we'll want to try a full uninstall and reinstall of The Sims.
We'll then want to check a few things on your controller:
- Replace the batteries in your controller
- Manually power-cycle your controller
- Connect your controller to your Xbox using a USB cable
- Check for a firmware update on your controller
- Restore your controller's button mapping settings to default-
- Press the Xbox button to open the guide.
- Under Profile & system, select Settings.
- Under Devices & connections, select Accessories.
- On the Xbox Wireless Controller screen, select Configure.
- Select Restore to default to set the controller back to its original button mapping.
Please also test a different controller, and keep me updated on how this works for you!

So first I tried uninstalling my game and all of my packs and then I updated my controller and the bug is still occurring. I didn’t explain the actual issue well in my first post but basically as you can see in the screenshot when I hover over sims in game the white outline does not appear I can only see their name and life stage, the same problem on objects, when I press A on sims/objects no options show up I hear the click sound effect but nothing appears. Also when I click LT to switch between sims, my screen is dragged to the other sim but I’m not playing her if that makes sense, I can’t switch to her at the bottom.

Hi, I thought I should let you know that I have tried different households in my save and they worked fine, I could play as normal with no bugs and so I tested the household I was playing originally again and it was the same bug. I’m sorry I don’t mean this to rush a reply from you at all I just thought that I should let you know in case this helps understand the problem

I've merged all your posts together as it sounds like you're seeing corruption in your current game save if these bugs are not occurring on a brand new save file. These issues can happen with very old save files and save files with a lot of played households.
If the save file has become corrupt, we wouldn't be able to restore it, as these files are stored locally to your console, however we can move your Sims over to a brand new save file instead so you can continue playing with your families. You can upload your current played families into the Gallery and then download them into a brand new save file and play with them there- do the same for any lots or builds you want to carry over to the new save file as well. However, doing this will reset these Sim's relationships with anyone outside of the household, and will remove any items in their inventories.
