My Sims 4 keeps freezing and has slow loading screen, help....

So, I have the sims 4 game on console and I have most of all of the expansion packes, game packes, stuff pack and some of the kits. The problem I having is my game will freeze every 10-15 mins and when ever travel my sims the game will freeze or have very slow loading screen. Please help it has been going on a year now.
