Have you, or someone else, ever changed the default settings in Accessibility or in the Mouse/Trackpad/Keyboard settings in System Preferences?
Does it happen in a new clean game? Move your Sims 4 folder from Documents > Electronic Arts to the desktop then launch a new game.
