Sometimes your sim can be showing it is ready for a promotion based on the job performance bar and meeting the requirements, but it can still take a few times going to work with your sim needs and health all in green and sometime with a specific emotion, like happy, inspired or focused depending on the career.
I didn't understand the rest of your post, sorry. Can you explain more?
