Doctors can't do surgery/xrays - menu option doesnt appear

Product: The Sims 4
Platform:PC
Which language are you playing the game in? English
How often does the bug occur? Every time (100%)
What is your current game version number? UPDATE: 01/16/2020 – V1.21
What expansions, game packs, and stuff packs do you have installed? All of them
Steps: How can we find the bug ourselves? Doctor career, sims no longer have the option to: 1) admit patients to bed 2) refer for x-ray/treadmill 3) perform surgery 4) deliver babies. The social menu no long has the refer patient option.
What happens when the bug occurs? Doctor career, sims no longer have the option to: 1) admit patients to bed 2) refer for x-ray/treadmill 3) perform surgery 4) deliver babies. The social menu no long has the refer patient option. The other options like diagnose and examine and treat are still there, but they're greyed out since i cant admit them to exam beds to do any tests.
What do you expect to see? I'd like this menu option to come back so i can advance this sim more. Preferably I'd like it to work on my original save file so I dont have to start over 100% from scratch.
Have you installed any customization with the game, e.g. Custom Content or Mods? Not now. I've removed them.
Did this issue appear after a specific patch or change you made to your system? No

I backed up my game and made a new save and it still happened with the brand new sim and save file. It was weird because it was working fine, then the next day they went to work it just disappeared and now i've spent hours trying to fix it/figure it out so I dont lost my big save!

    @shlotatoThough are disabled until after you do some of the basic diagnostic, once your close other menu could have came up. Also some patients may have already had x-rays done and so that will be greyed out. Also make sure you check the x-ray machines if they are broken they will be greyed out. Check the tooltip for the greyed out actions to why you can't access them. It will say "machine is currently broken", or "patient already had that done." I am not in the game so my wording might not be exact.

    Edit: I almost forgot you need to be a Doctor to Unlock the other machines.

    @shlotato Which level of the career are you at ?

    I've tested a save before that had issues with admitting Sim to the bed and something was off with that but can't remember exactly what it was.

    If your Sim is at the appropriate level for all those things, is the Sim an occult ?

    I just played this the other day to test a different issue and had all the options available,
