Game keep crashing on intro loading screen

I've had this problem for a couple months now and I can't seem to figure out what's wrong. When I try to start up my game it'll freeze on the opening logo. And then if I go to a different window and come back it'll switch to a completely white screen and then eventually crash and give me an initialization error at start up notification. I've tried taking out my mods folder but its the same thing every time.  It doesn't generate a lastcrash file but I have my dxdiag attached. I have an interactive desktop wallpaper and I've noticed that it also freezes when I try to run the game.  The game was running perfectly before all this and I don't remember changing anything specific. 

I also looked into my critical events and there wasn't anything directly related to the application but it generated a windows hardware error at the time of the game crash:

I don't know what else to do at this point and I'm scared it might just be an error with my PC so any advice helps!
