I'm having the same issue. I don't use mods, only custom content. Or does that count as mods?
It's really frustrating. I've been using mods since I got this game in 2014, and this issue appeared only recently.
First all the career rewards were unlocked, and now only a couple of them.
I'd really love to resolve this issue, doesn't feel right to have items I haven't unlocked yer.

I just checked - now ALL of my career rewards are unlocked. I did not install any mods in the last couple of months, so I don't think that plays a role? Maybe the items unlocked right after my last update, which was yesterday, Most of the items were still locked last week (most of them).
Unlocked items include the Growfruit challenge planters, even though I haven't even started that challenge. Also items like One Single Rose even though I've never let any of my sims go on a date, as well as items from careers I haven't played yet (criminal, business, tech, etc).
Any ideas on what it could be or how to fix it? I reinstalled my game on a new laptop last month, so that didn't fix it. It's been like this for a while now, just didn't think to look for a fix until now, when I saw I already unlocked the planters somehow.
I also don't use cheats, except for moveobjects and freerealestate for unplayed sims. I read that there is a cheat that lets you unlock career rewards (that is about the only result I got when looking for the answer on Google), but is there a way to reset or lock them again? Maybe that could fix the issue.

Not sure if this will apply in your case but these items are unlocked if you are in build/buy on an unoccupied/unplayed lot, by design. They shouldn't be unlocked for a household you are actively playing. I didn't realize this until I read that post and think that's a nice feature.
Genius!! I hadn't realized the items were unlocked only when in build mode on an unplayed lot, it was never like this (well, a year or so ago anyway).
