I tried it with out mods and still have the same problem where cas don't work and have to ALT SHIFT DEL to close out the game

This is most probably caused by a mod (one or multiple). Try the method below to narrow down your problem. If it's a mod, you can use the 50/50 Method explained here: http://modthesims.info/wiki.php?title=Game_Help:Finding_Problem_Custom_Content. There's also lists you can find by googling that tell you which are incompatible

Try resetting your user files as described in the factory reset described in this post: http://answers.ea.com/t5/The-Sims-4/FAQ-Troubleshooting-Steps-amp-Known-Issues/m-p/3621199
You won't loose anything. Just make sure to not delete any files and make a backup.
Try on a new game with nothing added back first (start new game). If it works, put back your save game and try again. If it still works, you can start putting stuff back.
- Options.ini (or just re-set all your settings manually)
- Tray Folder
- Screenshots/Videos/Custom music
- Rest of the save games (leave out slot_00000001.save which is the autosave)
- Custom content like clothing, hair, skins, make up ....
- Mods (one by one, making sure they all still work)
The rest does not need putting back, cause they are mostly cache files that recreate or log files that are not needed for the game.
If that doesn't work, try repairing the game through Origin
The Issue is with Cas colmns v1.4 remove that and ur Create a Sim will work again i had it install so i would have extra colums is cas but after the update it caused an issue I guess.
I'm not sure this is the issue, b/c I never had this mod and I still have this problem.
I removed extra columns Mod....This was the problem with me, Thank you for answering KAZUL73!

Yes, I am having this same problem and I deleted all mods that popped up as conflicting, and my CAS still wouldn't work. One that was not included in the conflicting mods list was, in fact, the more columns mod. I think this is the source of the issue, so I am going to try and delete then see how it goes.
