After removing mods, did you try with a new Sim or the same one ? How did you age them up ? Cheat ? Cake ?
Does it happen to any of the teens you tried ?

@kaitlinsasafras : Did you delete the localthumbcache.package from your Sims 4 folder in Documents and run a Repair in Origin?
Also, I know bad CC can cause unexpected issues with Sims' appearances. (For example, certain CAS CCs cause new townie and NPC Sims to generate with the Grim Reaper's cartoony facial features...yes, there's a face under that hood, apparently!)
