@loubylou1983 What do you have for a power supply? (If you're not sure, you can just look at it.) lease list the manufacturer, wattage, and rating, i.e. Corsair 750W 80+ Gold; or just link the product page if you know what model you have.
The hwinfo log has some strange readings. For one, a couple of the PSU readings aren't present at all, and in fact they're the ones I'd most want to know about. Your graphics card runs fine on full load for a couple minutes, but then the wattage starts to fluctuate, suggesting there might be a power issue. It's difficult to tell though without a direct reading.
Your GPU fan also reports running at zero RPM, which is odd considering that it's doing a good job of keeping the card relatively cool. Still, it would be good to physically look at the fan and make sure it runs when the computer is on. Other than a couple of stranded sensors that don't seem to be connected to anything, all the temperature readings look fine.
For the case fan, it's not unusual to have one or more fans plugged into a hub which is plugged directly into the PSU. It's still better to have the fan plugged into the motherboard as well though. It should have a small cable with an equally small four-pin connector that you can plug into the board's case fan header. Look for the one labeled with a v on page 14 of your motherboard's manual:

I'm not sure this is a power issue though. Since the dxdiag just lists a bunch of the same errors with the MSI software, please try to play again, let the game crash, then look for an associated entry in the Relibility Monitor. Hit Windows key-R and enter "perfmon /rel" without quotes, and you'll see a chart of errors and updates with a column for each day. Today is all the way on the right. Please click "view technical details" on the most recent entry, double-check its timestamp (specifically within the technical details) to make sure it is in fact from when the game crashed, copy the info, and paste it here.
