Sims completely ignore commands

Specifically when telling them to deal with infants/toddlers (IE changing diapers at changing tables) when you tell your sim to do something they will stand still for a few seconds then pop up with a new different task and completely skip over what I've told them to do. Like when I tell them to change the baby's diaper at the changing table they will override it and put the infant in the crib instead even if there are 3 to 4 tasks telling them to change the diaper, or they will be in the middle of doing something like going to the bathroom or sleeping and they'll get up just to go do some mundane task instead. I've tried resetting them to debug them and it doesn't change how they interact at all. It's super annoying.
