When I played Sims 4 I got the unknown skin.
The Unknown Skin, It's red and black, and I do not know why it had a question mark over it while the character model was moving. It quite scared me, honestly.

DaedrlcPrincess This is most often caused by broken default skin mods, broken overlay mods, or broken sliders, and it can happen on specific Sims you've downloaded from somewhere if that Sim was made with one of those originally.
If it's only on certain Sims, the only fix is to delete them.
If it's on all Sims, try removing all your skin mods, skin overlays, and sliders (don't forget to delete localthumbcache!) and then reload. You can add them back in gradually to find problem ones, and also check Previously Broken mods and CC
