The first couple of posts in this thread from ‘16 have the answer.
To paraphrase, the player has no influence over the toilet bar.. It’s going to stay red until game coding “decides” the baby has messed its diaper. The bar will then show green. I’ve had it take 2+/- weeks for this to happen but I don’t know the actual length of time.
Ideally, the bar should act like the toilet gauge for all other ages- green until the diaper is dirty and the ‘change diaper’ option should not appear until dirty as well.
