For a bath - babies cannot be bathed. Only todldlers. To clean a baby and turn the hygiene bar green you need an adult sim change the diaper. As far as the toilet bar goes there is nothing you can do to make that green. The baby has to go in its diaper, then the bar will turn green. And that is when you will need to change its diaper because the hygiene bar will become red.
