It's both player level (referred to as rank but is not player position, but level such as Hand of Kane and defeat of a fortress.
My vote would be for the level to be raised and the Fortress kill needing to be 1st, 2nd or 3rd which would discount many especially those that stick around on a world and 12th place badge because everyone else has left!
Also of note to some, killing the Fortress on the PTE and rank/level on the PTE don't count!
