I've noticed this happen on a few worlds in the past and as noted either the credits were returned on logging in and no inventory added or inventory was added (and credits deducted) sometimes multiple times as I had tried to use inventory a few times before I realised it wasn't working. In all cases I have put it down to a breakdown in communications between one system and another. Sometimes I have also noticed it coincides with low fps rates.
