I have a CSW v2.5 (in PC mode) with the CS Universal Hub v2 on PC.
I too have the same problem of spontanious disconnects with the "Steering input device has changed" message. It happens to me with different intervals (so no fixed time between). The shaft has no play while racing, so I guess that can't be the source of the problem. No other game (PCars 1/2, Dirt Rally 1/2, Assetto Corsa) I play has the same issue.
Tried something last night that seems to work:
While in the game (in my case in the main menu) i took the wheel off the base and reseated it again (firmly). I even added the locking bolt later.
With or without the locking bolt in place the problem didn't occur anymore.
I guess this game is very sensitive to very minute connection losses, where all the other games are not. I can't come to a different conclusion than this one.

does not work on PS5 unfortunately. I have QR2 which is rock solid and detaching/attaching the wheel does not improve anything, the game still constantly switches between the multiple recognized wheelbases.
