Campaign Challenges are Phantom "tracked"

Please fix this issue. My "tracked" challenges show that I have 4/4 challenges tracked. However when I go over to the section, I only have 3 of them. This started happening when I completed the challenge chain for the campaign. It stayed behind and it's really ruining the multiplayer experience for me.
