I think this is the same issue I've found. In my manager career, my starting CAM has played really well, a +4 to his OVR by the end of the first season. Earlier in the season, he asked for a contract renegotiation - I happily obliged - he was happy to take a £500 per week wage increase. About midway through the season, he has asked again - a bit greedy, but figured I could just bump the contract up again, but I got that `You have recently negotiated a new contract with this player.` error message.
Just now, near the end of the season, I got the message saying he's disappointed that I haven't entered the contract renegotiations with him - I still can't do it. Without warning, he is now listed as `Submitted a transfer request`, and I have no **bleep**ing control over it?

Really hoping this bug is one that they actually work on instead of Ultimate Team for once :(
