Coaches not refreshing in manager career mode

Started a manager career mode with Bristol City in the EFL Championship but after getting promoted to the Premier League, the coaches don't refresh any more. A new coaching slot was added after starting the second season, but am now at the January transfer window in season #2, and no new coaches have appeared this entire season. Perhaps a bug associated with getting promoted?

I've tried to fire coaches and rehire, start a new save game, etc. and nothing seems to fix this.
