Season Pass Progress Reset — Missing Rewards for Levels 39–40

I've had an issue with my Season Pass rewards. I had already claimed the rewards for Season Pass levels 1 through 38.

After that, I completed additional objectives/missions to earn SP so I could unlock the remaining rewards for levels 39 and 40.

However, I was surprised to find that my Season Pass progress was reset back to the beginning, and I did not receive the rewards for levels 39 and 40.

Could you please investigate this and restore my Season Pass progress and/or grant the missing rewards?

I've paid for the SP Premium and this what I get? This is frustrating.
