For me, the issue happened with any challenge that was started. I was joining random players, so the specific challenge varied, but every one of them caused the same problem.
It seems like the issue might be connected to the "invisible / lagging party member"-bug. In all the sessions where the challenges failed to start, the other players I joined were either lagging heavily or completely invisible. After restarting the game several times and joining different players, I eventually ended up in a session where the other player was visible and not lagging, and with that player, every challenge we tried worked normally (including challenges that had previously caused the issue).
I also tried disabling cross-play to see if it made a difference, but from what I could tell it didn’t affect the issue. Challenges still failed, and people were still invisible or lagging.

Meaning that I noticed it will only occurred when I have low connection to the server (stuttering players in my world) and I dont let all the mission goals load in the list on the left side before the mission starts.
