Mods just disappearing when I equip them and have the game.

My mods continue to get erased after equipping them and having the game for a moment or any restart of the game itself. Also in certain instances it will make me restart the game after equipping mods and hitting confirm. It’s been 2 weeks of this. I’m a returning player.
