Tier II of Rotta the Hutt requires Teva be at Era Level 95. Mine is 5* and Level 95 but the prerequisite box still isn't checked.

Have you tried to reboot the game?
I did plenty of times. Logged in and out on both mobile and pc. It even checked off the R5 requirement box after upgrading to 5 stars but left the Teva one unchecked.
But.... I ended up uninstalling and re-installing the game and that fixed it.
