I have been having this issue for a very long time ( maybe years) where my sim gets a fear, I "cure" it and it pops up again the next day, or at the wrong time ( like a fear of the dark when the sim time is noon). I have tried turning the wants and fears off but it wont work.

Raesthetic has a mod called No More Wants, Whims and Fears which fixes this issue.

I'm not entirely certain if this is Base Game related or Life & Death related, but I added Clement Frost to my household (by having him get married to a preexisting member) and he immediately showed Wants & Fears bubbles above his character icon even though I had that setting disabled (unchecked).
I rechecked it in settings, applied changes, then unchecked it again, and applied changes again (without exiting the Game Options menu) to get rid of them, but when he aged up to an Elder it happened again. Specifically just a want to "Designate an Heirloom" which makes me think it may be Life & Death specific instead of Base Game. I haven't seen this behavior before when adding NPCs to households so I really lean toward it being a Life & Death issue.
I did have an infant in the household be aged-up by having an adult help them blow out candles on a cake and the now-toddler did not have Wants or Fears bubbles appear so I think this (the Wants & Fears setting not being honored) is exclusive to NPCs.
Screenshots are attached showing Clement Frost having the Want to "Designate an Heirloom" with a reason of "Aging Up" as well as the Gameplay tab showing the "Show Wants & Fears" option is unchecked while that bubble is displayed.

I turn off Wants and fears because I’m a casual here for designing homes. However after the July 1st update I’m getting Wants and Fears moodlets with it toggled off. Tense and Anger because of missing out moodlets, but I made sure it was toggled off. Just thought it should be put on the radar. Not a total big deal, but note worthy nonetheless.

console currently has an issue with preferences resetting.
Did you check it's toggled off while playing a save game ?

I did multiple times. Toggled on and back off in play mode. However my sim was “very angry” about missing out still.

For me, this has been happening ever since Wants and Fears was introduced.
I set Wants & Fears is always disable.
Other my play settings:
Autonomy is full on
Aging is all Sims off (aged if necessary)
Automatic Death Inventory Handling is on (as Neighborhood Stories is on)The following things is I saw in my game.
When NPCs join the household, Wants popped up on their head. This happens evrytime NPCs join the householed and only happens to joined NPCs. But I can remove those pop up by resetting from Game settings.
Except the above, Wants pop up happen only about romantic interactions and that happen there are Sims have romantic relationship near by. - 934759230 days agoSeasoned Novice
Hi. I have been noticing for some time now on both the PC and the PS5 that when I travel, I will have a want or fear showing upon arrival, although I have them turned off. Also, I had gone to Manage Worlds, then started playing my sim again, and the same thing happened, BUT when I traveled to the café in the Ranch World, it did not happen, and the bartender did not walk away - except that they seemed to change shifts at 6:30 pm. So, there is sadly some intermittent aspect.
This started out only happening in Ravenwood right after that was released (I pre-ordered that one), but now it is a constant thing in several worlds, if not all worlds. I have not tested in all worlds. My impression is that it got worse after I bought the hobby pack.
In the screen shot, you see two issues:
- The bleed-through want or fear.
- That the bartender has simply walked to the corner. I ordered food, and it cancelled the action, because the bartender simply walked away. The same thing happened in the cottage world, but not in the café in the Ranch World.
The character on the PS5 was a newly created character (for the hobby pack), and the PS5 has no mods. The PC is modded. It could be a mod, if it were not also happening on the PS5, which is unmodded. So, unlikely to be a mod causing this.
The only fix is to constantly open the game options, turn on the wants and fears, apply, then turn them off again, and apply again.
We had this problem a few years ago, and at some point it was fixed. The issue seems to be that somehow the wants and fears setting is being ignored or changed, but somehow only partially. Only 1 item shows. I have not looked to see what they are. It's just an annoyance, when you have turned them off. Sadly, the wants and fears do not take into account the character's traits or age, or how far they have already progressed at university or in their jobs, so it became an undesirable aspect of the game that seemed to exist in some separate, parallel reality, where my sim was someone completely different.
We have that same issue with the conversation choices. We say our sim hates comedy and mischief, also deception (needs to be done in a few places in sim creation mode) - autonomy is completely off generally and for the active sim, but the game still forces conversation topics that involve these things. That would be a matter of turning off auto-conversation when autonomy is turned off, which would be super, but is a separate issue. :-)
Please check the flags in the wants and fears to assure that when they are 0 that they stay 0 (I assume a simple binary model). That traveling or loading a game does not do something to switch them to 1.
Thank you! We all know that this is a wonderfully complex game, and it's not possible to get everything working correctly all at once.
